Service Agreement

Siha Care (Siha Technologies Pvt. Ltd.) continuously strives to create excellence in the service delivery model by keeping people-centricity at the core, technology-led processes & innovation, and consistently delivering the best possible clinical outcomes to every patient we service. Cooperation from the patient and family members is of utmost importance in achieving this mission. Please review this agreement carefully, as it describes the understanding between you ("Patient") and Siha Care ("Agency") regarding the services you have requested. Our authorized representative can provide clarifications on the content of this agreement.



This document (this “Agreement”) is made and entered into on 25 August 2024 (the “Effective Date”) by and between the patient (hereinafter referred to as the “Beneficiary”) and Siha Technologies Pvt. Ltd. (from now on referred to as “Siha Care”), whereby Siha Care agrees to provide the homecare services as requested by the beneficiary in the manner and upon the terms and conditions set forth in this Agreement.



I hereby confirm that I (the patient) agree as follows:

Term of Agreement - The term of this agreement will start on the Effective Date and will continue on an as-needed basis until the Agreement is terminated by either party, as provided hereunder.



Services Requested:

- Caregiver (CG)
Basic assessment and vital monitoring (Temperature, Pulse, Respiration, Blood Pressure, Glucose monitoring)

Hygienic care (Hair Care, Nail Care, Skin Care, Back Care, Assistance to Bathroom, Assisted Bath, Bed Bath, Mouth Care, Bed Making, Use of Multifunction Bed)

Elimination needs management (Diaper Change, Offering Bed Pan, Offering Urinal Pan, Urine Bag Emptying, Care of Urobag)

Nutritional needs assistance (Helping in Oral and RT Feeding)

Ambulation and exercises (Assistance in Position Change, Assistance Walking, Wheelchair Mobilization, Simple Daily Exercises, Range of Motion Exercises, Spirometry)

Psychological support and diversion therapy (Assistance in Reading, Spiritual Support, Engagement Activities)

Basic nursing procedures (Hot & Cold Application, Steam Inhalation, Nebulization)

We will provide the services requested and agreed upon as set out in the Care Plan enclosed. The preferred day, time, and duration of services will be mutually agreed upon by you and/or your representative.



Related to the Selection of Services

a. I/Patient’s NOK was explained clearly about the services in detail by the Siha Care representative.

b. The differences between different categories and their deliverables.

c. Shift timings are as mentioned and agreed upon during onboarding.

d. Every admission in Siha Care will undergo a standard process of care assessment, hand-holding, and supervisory visits as per standard protocols.

e. Staff will be allotted for each patient as per care requirements. To cover during the assigned staff’s week offs and emergency leaves, duties will be assigned to other staff as per availability.

f. During unavoidable situations, services will be kept on hold for the said time and resume once alternative sources are arranged.

g. Services will be delivered as per booking and service agreement.



Related to Payment for Services

a. The estimated amount of INR ₹29,999 per month will be paid in advance, with recurrent deposits for the next cycle of services.

b. An extra amount will be charged for the continuation of the same staff during their off/leaves/Public Holidays.

c. Care requirements may increase or decrease based on the patient’s condition.

d. An extra amount will be charged for consumables used by the staff to provide care.

e. Transportation charges may vary if the patient travels to different locations and would like the staff to accompany them.

f. During the service, if the staff is injured for any reason, the family will take complete responsibility for the treatment of the staff.

g. It is the responsibility of the family to provide all the required conditions for efficient equipment usage.

h. All charges due should be paid before the time of discharge of the patient.

i. In case of non-payment by the patient/NOK, Siha Care has the right to stop services until payment is received. Patients are regularly updated about their payment status as per availed services.



Payment Mode:

- Payments should be made via online transfer to Siha Care.

- No cash payments will be accepted directly by the staff unless authorized by appropriate personnel.



Monitoring and Follow-up:
I/Patient’s NOK understand that my service requests/needs will be reviewed by the Supervisor as needed, and that the service(s) may be changed according to requirements.



Your signature and/or your representative's signature below indicate that you and/or your representative have read, understand, and agree with the terms and conditions of this Service Agreement.



Medical Records:

At the end of the service, the Nursing Supervisor will be responsible for collecting the relevant data, and you/your representative will be provided with a closure report.

General Terms and Conditions for Siha Care Services

1. Agreement Term: This Agreement shall come into force on the Effective Date and shall remain in force for a period of one year from the date of signing or until the date the services are availed, whichever is earlier, unless extended by mutual consent or terminated in accordance with the terms hereof.

2. Payment Terms: In consideration of the provision of Services hereunder, payment of INR ₹29,999 shall be paid by the Beneficiary only in advance by card/online payment to Siha Care.

  • No cash will be paid directly to the staff, unless authorized by appropriate personnel.

  • The professional service fee quoted herein is inclusive of all taxes payable by the Siha Care staff in any jurisdiction but exclusive of goods and services tax.

  • All charges due should be paid before the time of discharge of the patient.

  • In case of non-payment by the patient/NOK, Siha Care has the right to stop services until payment is received. Patients are regularly updated about their payment status as per availed services.

3. Service Provision:

  • The Siha Care staff will only render those Services which have been expressly agreed upon by the Parties under this Agreement. No variation of this Agreement shall be valid unless it is in writing (which, for this purpose, does not include email) and signed by or on behalf of each of the Parties.

  • During the term hereof, all the basic amenities (such as proper boarding and lodging facility) required for stay at the Beneficiary premises will be provided for the staff, at the Beneficiary’s sole cost and expense.

  • The Siha Care staff will not perform any domestic and/or household chores such as cleaning, cooking, etc.

  • Regular food and beverages must be served to the Siha Care staff at appropriate times and in adequate quantity. Adequate resting/sleeping time for the Siha Care staff is to be ensured.

  • The Beneficiary and their family members shall treat the Siha Care staff with due respect.

4. Leave and Replacement:

  • As per extant Indian labour laws, the Siha Care staff shall be eligible for one day of leave for every 15 days of Services rendered. In case the employee is not availing the leave and extends services, they will be suitably compensated.

  • During the term of this Agreement, Siha Care reserves the right to replace the Siha Care staff engaged in the Services, provided that such staff is replaced with another who is similarly qualified. If the Siha Care staff is not found suitable for valid reasons, the same will be brought to the attention of Siha Care who will make efforts to find a suitable replacement within 7 working days.

5. Termination:

  • Either Party may terminate this Agreement for convenience by giving a prior written notice of 7 days to the other Party or at any time with the mutual consent of the Parties.

  • Refund will be credited within 7 days to your original payment method.

  • Either Party may terminate this Agreement forthwith if the other Party fails to perform its obligations hereunder or is in material breach of any terms and conditions of this Agreement.

  • In case of early termination of this Agreement, all payments owing from Beneficiary to the staff, or refunds due from the staff, shall become due and payable, and legally enforceable, upon termination hereof and balance advance, if any, will be refunded on a pro-rata basis as per the lock-in term mentioned in the Financial Counselling Form (FCF).

6. Non-Solicitation: The Beneficiary or their family members shall not, during the term hereof and for 12 (twelve) months after the expiry or termination hereof, either directly or indirectly, either on their own behalf or by or through any other person, hire, solicit, canvass, or entice away the Siha Care staff for itself or any other person or entity. In case of any breach hereof, damages equivalent to three months payment will be recovered from the beneficiary.

7. Termination Due to Death: This Agreement shall only be valid till the Beneficiary is alive. In case of the death of the Beneficiary during the term hereof, the Agreement shall be deemed to have expired immediately. No refund can be claimed or made in case of death or shifting to another location.

8. Governing Law: This Agreement shall be governed by and interpreted in accordance with the laws of India. Any dispute arising out of or relating to this Agreement shall be subject to the jurisdiction of the courts in New Delhi.

9. Safety and Liability:

  • Safety is the Beneficiary’s highest priority, but the Beneficiary acknowledges that the Services performed hereunder contain inherent risks of infection, injury, or death. By agreeing to receive the Services, the Beneficiary agrees that he/she accepts these risks completely and shares in the responsibility to maintain safety by his/her own actions or conduct.

  • Siha Care shall in no circumstance be liable to the Beneficiary as a result of any infection, injury, or death as a result of providing the Services during the term hereof.

  • Siha Care shall not be liable to the Beneficiary for infection, injury, death, any direct loss or damage including financial loss, or for any indirect, special, or consequential loss or damage. The Beneficiary hereby forever discharges Siha Care from all actions, causes of actions, suits, debts, obligations, claims, and demands.

  • Regardless of cause, the Beneficiary shall be liable for and indemnify Siha Care from and against any and all claims arising out of personal injury, illness, infection, death, or property loss or damage suffered by the Beneficiary, his/her next of kin, or any person in the Beneficiary’s premises.

10. Authorization Consent Form for Staff Services:

  • The Beneficiary understands that appropriately qualified health care personnel will provide care. The Beneficiary has the right to refuse treatment or terminate services at any time by notifying Siha Care in writing.

  • The Beneficiary confirms that Siha Care has provided information about the duration and nature of proposed treatment/procedure/service and any expected side effects.

  • The Beneficiary has been given the opportunity to ask questions about the treatment/procedure/service, and these have been satisfactorily answered.

  • The Beneficiary understands the nature of the recommended treatment/service/procedure and the risks involved and willingly agrees to it.

  • The Beneficiary will be responsible for the prognosis if hospitalisation is advised and refuses the same, and accepts full responsibility for the outcome.

  • The Beneficiary has been informed of any on-call service available and authorizes Siha Care to take equal responsibility for shared services.

  • The Beneficiary indemnifies Siha Care against any claims made because of the treatment, which has been accepted voluntarily with full awareness of risks.

  • The Beneficiary authorizes Siha Care to release copies of medical records or summaries to other health care providers or regulatory bodies for continuity of care and quality assurance.

  • The Beneficiary authorizes Siha Care to record all conversations/interactions about the services provided if deemed necessary.

  • The Beneficiary authorizes Siha Care to use registered email/phone number/address for sending documents and communications.

  • The Beneficiary confirms that Siha Care is offering recovery and rehabilitation services as per diagnosis from primary hospital.

  • The Beneficiary voluntarily opts for the treatment/service and assumes responsibility for any consequences.

  • The Beneficiary agrees to allow Siha Care to use photos/videos for research, educational, and promotional purposes.

  • In medical emergencies, the Beneficiary authorizes Siha Care to provide or obtain medical treatment and assumes sole responsibility for all charges.



Signature:

  • The signature of the Patient or their representative indicates that they have read, understand, and agree with the terms and conditions of this Service Agreement.

Medical Records:

  • At the end of the service, the Nursing Supervisor will collect the relevant data and provide a closure report to the Patient or their representative.

Responsibilities of a Caregiver

  • Assist with Daily Living:

    • Aid patients with getting out of bed including personal hygiene routines like oral care, nail care, bathing, dressing, and grooming.

    • Help with repositioning, positioning, and changing diapers as needed.

    • Support with mobility, including walking, transferring, and using mobility aids.

  • Medication Management:

    • Administer prescribed medications as per the schedule.

    • Remind family members in advance when medicines are running low to ensure timely refills and continuity of care.

    • Monitor for and report any side effects or medication-related issues to health manager and family

  • Vital Signs Monitoring:

    • Measure and record vital signs such as blood pressure, heart rate, temperature, and oxygen saturation.

    • Monitor and document changes in health indicators.

  • Nutritional Support:

    • Prepare and serve meals and snacks based on dietary restrictions and preferences.

    • Ensure proper hydration and nutrition throughout the day.

  • Emotional and Social Support:

    • Provide companionship and engage in activities that promote mental and emotional well-being.

    • Address any fears, concerns, or emotional needs of the patient.

  • Document Care Activities:

    • Maintain detailed and accurate records of care provided, including vital signs, medication administration, meals served, and patient observations.

    • Track and document symptoms and changes in the patient’s condition as outlined in the care plan

  • Safety and Emergency Preparedness:

    • Maintain a clean and sanitary living environment.

    • Schedule reminders for the regular changing of catheters and RT tubes based on care plan.

    • Check the RT tube regularly for any signs of blockage or leakage and ensure that it is functioning properly.

    • Immediately report any displacement of the Rt tube to the health manager and family members.

    • Document any changes in wound condition and report to the healthcare team.

    • In case of an emergency, promptly notify the patient's family and assist them in seeking emergency medical support.

  • Communication and Reporting:

    • Communicate regularly with the patient’s family and healthcare team.

    • Provide updates on the patient’s condition and any notable changes or concerns.

  • Support with Rehabilitation:

    • Assist with prescribed therapeutic exercises and rehabilitation activities as directed by healthcare professionals.

    • Facilitate participation in activities that improve physical and cognitive health.

  • Assist with Appointments:

    • Accompany patients to medical appointments and ensure they receive necessary follow-up care.

  • Overnight Care (if required):

    • Provide support during nighttime, including turning, repositioning, and managing incontinence if needed.

Responsibilities of a Caregiver

    • Prepare and serve meals and snacks based on dietary restrictions and preferences.

    • Ensure proper hydration and nutrition throughout the day.

  • Emotional and Social Support:

    • Provide companionship and engage in activities that promote mental and emotional well-being.

    • Address any fears, concerns, or emotional needs of the patient.

  • Document Care Activities:

    • Maintain detailed and accurate records of care provided, including vital signs, medication administration, meals served, and patient observations.

    • Track and document symptoms and changes in the patient’s condition as outlined in the care plan

  • Safety and Emergency Preparedness:

    • Maintain a clean and sanitary living environment.

    • Schedule reminders for the regular changing of catheters and RT tubes based on care plan.

    • Check the RT tube regularly for any signs of blockage or leakage and ensure that it is functioning properly.

    • Immediately report any displacement of the Rt tube to the health manager and family members.

    • Document any changes in wound condition and report to the healthcare team.

    • In case of an emergency, promptly notify the patient's family and assist them in seeking emergency medical support.

  • Communication and Reporting:

    • Communicate regularly with the patient’s family and healthcare team.

    • Provide updates on the patient’s condition and any notable changes or concerns.

  • Support with Rehabilitation:

    • Assist with prescribed therapeutic exercises and rehabilitation activities as directed by healthcare professionals.

    • Facilitate participation in activities that improve physical and cognitive health.

  • Assist with Appointments:

    • Accompany patients to medical appointments and ensure they receive necessary follow-up care.

  • Overnight Care (if required):

    • Provide support during nighttime, including turning, repositioning, and managing incontinence if needed.




Service Agreement

Siha Care (Siha Technologies Pvt. Cooperation from the patient and family members is of utmost importance in achieving this mission. Please review this agreement carefully, as it describes the understanding between you ("Patient") and Siha Care ("Agency") regarding the services you have requested. Our authorized representative can provide clarifications on the content of this agreement.



This document (this “Agreement”) is made and entered into on 25 August 2024 (the “Effective Date”) by and between the patient (hereinafter referred to as the “Beneficiary”) and Siha Technologies Pvt. Ltd. (from now on referred to as “Siha Care”), whereby Siha Care agrees to provide the homecare services as requested by the beneficiary in the manner and upon the terms and conditions set forth in this Agreement.



I hereby confirm that I (the patient) agree as follows:

Term of Agreement - The term of this agreement will start on the Effective Date and will continue on an as-needed basis until the Agreement is terminated by either party, as provided hereunder.



Services Requested:

- Caregiver (CG)
Basic assessment and vital monitoring (Temperature, Pulse, Respiration, Blood Pressure, Glucose monitoring)

Hygienic care (Hair Care, Nail Care, Skin Care, Back Care, Assistance to Bathroom, Assisted Bath, Bed Bath, Mouth Care, Bed Making, Use of Multifunction Bed)

Elimination needs management (Diaper Change, Offering Bed Pan, Offering Urinal Pan, Urine Bag Emptying, Care of Urobag)

Nutritional needs assistance (Helping in Oral and RT Feeding)

Ambulation and exercises (Assistance in Position Change, Assistance Walking, Wheelchair Mobilization, Simple Daily Exercises, Range of Motion Exercises, Spirometry)

Psychological support and diversion therapy (Assistance in Reading, Spiritual Support, Engagement Activities)

Basic nursing procedures (Hot & Cold Application, Steam Inhalation, Nebulization)

We will provide the services requested and agreed upon as set out in the Care Plan enclosed. The preferred day, time, and duration of services will be mutually agreed upon by you and/or your representative.



Related to the Selection of Services

a. I/Patient’s NOK was explained clearly about the services in detail by the Siha Care representative.

b. The differences between different categories and their deliverables.

c. Shift timings are as mentioned and agreed upon during onboarding.

d. Every admission in Siha Care will undergo a standard process of care assessment, hand-holding, and supervisory visits as per standard protocols.

e. Staff will be allotted for each patient as per care requirements. To cover during the assigned staff’s week offs and emergency leaves, duties will be assigned to other staff as per availability.

f. During unavoidable situations, services will be kept on hold for the said time and resume once alternative sources are arranged.

g. Services will be delivered as per booking and service agreement.



Related to Payment for Services

a. The estimated amount of INR ₹29,999 per month will be paid in advance, with recurrent deposits for the next cycle of services.

b. An extra amount will be charged for the continuation of the same staff during their off/leaves/Public Holidays.

c. Care requirements may increase or decrease based on the patient’s condition.

d. An extra amount will be charged for consumables used by the staff to provide care.

e. Transportation charges may vary if the patient travels to different locations and would like the staff to accompany them.

f. During the service, if the staff is injured for any reason, the family will take complete responsibility for the treatment of the staff.

g. It is the responsibility of the family to provide all the required conditions for efficient equipment usage.

h. All charges due should be paid before the time of discharge of the patient.

i. In case of non-payment by the patient/NOK, Siha Care has the right to stop services until payment is received. Patients are regularly updated about their payment status as per availed services.



Payment Mode:

- Payments should be made via online transfer to Siha Care.

- No cash payments will be accepted directly by the staff unless authorized by appropriate personnel.



Monitoring and Follow-up:
I/Patient’s NOK understand that my service requests/needs will be reviewed by the Supervisor as needed, and that the service(s) may be changed according to requirements.



Your signature and/or your representative's signature below indicate that you and/or your representative have read, understand, and agree with the terms and conditions of this Service Agreement.



Medical Records:

At the end of the service, the Nursing Supervisor will be responsible for collecting the relevant data, and you/your representative will be provided with a closure report.

General Terms and Conditions for Siha Care Services

1. Agreement Term: This Agreement shall come into force on the Effective Date and shall remain in force for a period of one year from the date of signing or until the date the services are availed, whichever is earlier, unless extended by mutual consent or terminated in accordance with the terms hereof.

2. Payment Terms: In consideration of the provision of Services hereunder, payment of INR ₹29,999 shall be paid by the Beneficiary only in advance by card/online payment to Siha Care.

  • No cash will be paid directly to the staff, unless authorized by appropriate personnel.

  • The professional service fee quoted herein is inclusive of all taxes payable by the Siha Care staff in any jurisdiction but exclusive of goods and services tax.

  • All charges due should be paid before the time of discharge of the patient.

  • In case of non-payment by the patient/NOK, Siha Care has the right to stop services until payment is received. Patients are regularly updated about their payment status as per availed services.

3. Service Provision:

  • The Siha Care staff will only render those Services which have been expressly agreed upon by the Parties under this Agreement. No variation of this Agreement shall be valid unless it is in writing (which, for this purpose, does not include email) and signed by or on behalf of each of the Parties.

  • During the term hereof, all the basic amenities (such as proper boarding and lodging facility) required for stay at the Beneficiary premises will be provided for the staff, at the Beneficiary’s sole cost and expense.

  • The Siha Care staff will not perform any domestic and/or household chores such as cleaning, cooking, etc.

  • Regular food and beverages must be served to the Siha Care staff at appropriate times and in adequate quantity. Adequate resting/sleeping time for the Siha Care staff is to be ensured.

  • The Beneficiary and their family members shall treat the Siha Care staff with due respect.

4. Leave and Replacement:

  • As per extant Indian labour laws, the Siha Care staff shall be eligible for one day of leave for every 15 days of Services rendered. In case the employee is not availing the leave and extends services, they will be suitably compensated.

  • During the term of this Agreement, Siha Care reserves the right to replace the Siha Care staff engaged in the Services, provided that such staff is replaced with another who is similarly qualified. If the Siha Care staff is not found suitable for valid reasons, the same will be brought to the attention of Siha Care who will make efforts to find a suitable replacement within 7 working days.

5. Termination:

  • Either Party may terminate this Agreement for convenience by giving a prior written notice of 7 days to the other Party or at any time with the mutual consent of the Parties.

  • Refund will be credited within 7 days to your original payment method.

  • Either Party may terminate this Agreement forthwith if the other Party fails to perform its obligations hereunder or is in material breach of any terms and conditions of this Agreement.

  • In case of early termination of this Agreement, all payments owing from Beneficiary to the staff, or refunds due from the staff, shall become due and payable, and legally enforceable, upon termination hereof and balance advance, if any, will be refunded on a pro-rata basis as per the lock-in term mentioned in the Financial Counselling Form (FCF).

6. Non-Solicitation: The Beneficiary or their family members shall not, during the term hereof and for 12 (twelve) months after the expiry or termination hereof, either directly or indirectly, either on their own behalf or by or through any other person, hire, solicit, canvass, or entice away the Siha Care staff for itself or any other person or entity. In case of any breach hereof, damages equivalent to three months payment will be recovered from the beneficiary.

7. Termination Due to Death: This Agreement shall only be valid till the Beneficiary is alive. In case of the death of the Beneficiary during the term hereof, the Agreement shall be deemed to have expired immediately. No refund can be claimed or made in case of death or shifting to another location.

8. Governing Law: This Agreement shall be governed by and interpreted in accordance with the laws of India. Any dispute arising out of or relating to this Agreement shall be subject to the jurisdiction of the courts in New Delhi.

9. Safety and Liability:

  • Safety is the Beneficiary’s highest priority, but the Beneficiary acknowledges that the Services performed hereunder contain inherent risks of infection, injury, or death. By agreeing to receive the Services, the Beneficiary agrees that he/she accepts these risks completely and shares in the responsibility to maintain safety by his/her own actions or conduct.

  • Siha Care shall in no circumstance be liable to the Beneficiary as a result of any infection, injury, or death as a result of providing the Services during the term hereof.

  • Siha Care shall not be liable to the Beneficiary for infection, injury, death, any direct loss or damage including financial loss, or for any indirect, special, or consequential loss or damage. The Beneficiary hereby forever discharges Siha Care from all actions, causes of actions, suits, debts, obligations, claims, and demands.

  • Regardless of cause, the Beneficiary shall be liable for and indemnify Siha Care from and against any and all claims arising out of personal injury, illness, infection, death, or property loss or damage suffered by the Beneficiary, his/her next of kin, or any person in the Beneficiary’s premises.

10. Authorization Consent Form for Staff Services:

  • The Beneficiary understands that appropriately qualified health care personnel will provide care. The Beneficiary has the right to refuse treatment or terminate services at any time by notifying Siha Care in writing.

  • The Beneficiary confirms that Siha Care has provided information about the duration and nature of proposed treatment/procedure/service and any expected side effects.

  • The Beneficiary has been given the opportunity to ask questions about the treatment/procedure/service, and these have been satisfactorily answered.

  • The Beneficiary understands the nature of the recommended treatment/service/procedure and the risks involved and willingly agrees to it.

  • The Beneficiary will be responsible for the prognosis if hospitalisation is advised and refuses the same, and accepts full responsibility for the outcome.

  • The Beneficiary has been informed of any on-call service available and authorizes Siha Care to take equal responsibility for shared services.

  • The Beneficiary indemnifies Siha Care against any claims made because of the treatment, which has been accepted voluntarily with full awareness of risks.

  • The Beneficiary authorizes Siha Care to release copies of medical records or summaries to other health care providers or regulatory bodies for continuity of care and quality assurance.

  • The Beneficiary authorizes Siha Care to record all conversations/interactions about the services provided if deemed necessary.

  • The Beneficiary authorizes Siha Care to use registered email/phone number/address for sending documents and communications.

  • The Beneficiary confirms that Siha Care is offering recovery and rehabilitation services as per diagnosis from primary hospital.

  • The Beneficiary voluntarily opts for the treatment/service and assumes responsibility for any consequences.

  • The Beneficiary agrees to allow Siha Care to use photos/videos for research, educational, and promotional purposes.

  • In medical emergencies, the Beneficiary authorizes Siha Care to provide or obtain medical treatment and assumes sole responsibility for all charges.



Signature:

  • The signature of the Patient or their representative indicates that they have read, understand, and agree with the terms and conditions of this Service Agreement.

Medical Records:

  • At the end of the service, the Nursing Supervisor will collect the relevant data and provide a closure report to the Patient or their representative.

  • The Beneficiary has been given the opportunity to ask questions about the treatment/procedure/service, and these have been satisfactorily answered.

  • The Beneficiary understands the nature of the recommended treatment/service/procedure and the risks involved and willingly agrees to it.

  • The Beneficiary will be responsible for the prognosis if hospitalisation is advised and refuses the same, and accepts full responsibility for the outcome.

  • The Beneficiary has been informed of any on-call service available and authorizes Siha Care to take equal responsibility for shared services.

  • The Beneficiary indemnifies Siha Care against any claims made because of the treatment, which has been accepted voluntarily with full awareness of risks.

  • The Beneficiary authorizes Siha Care to release copies of medical records or summaries to other health care providers or regulatory bodies for continuity of care and quality assurance.

  • The Beneficiary authorizes Siha Care to record all conversations/interactions about the services provided if deemed necessary.

  • The Beneficiary authorizes Siha Care to use registered email/phone number/address for sending documents and communications.

  • The Beneficiary confirms that Siha Care is offering recovery and rehabilitation services as per diagnosis from primary hospital.

  • The Beneficiary voluntarily opts for the treatment/service and assumes responsibility for any consequences.

  • The Beneficiary agrees to allow Siha Care to use photos/videos for research, educational, and promotional purposes.

  • In medical emergencies, the Beneficiary authorizes Siha Care to provide or obtain medical treatment and assumes sole responsibility for all charges.



Signature:

  • The signature of the Patient or their representative indicates that they have read, understand, and agree with the terms and conditions of this Service Agreement.

Medical Records:

  • At the end of the service, the Nursing Supervisor will collect the relevant data and provide a closure report to the Patient or their representative.

Responsibilities of a Caregiver

  • Assist with Daily Living:

    • Aid patients with getting out of bed including personal hygiene routines like oral care, nail care, bathing, dressing, and grooming.

    • Help with repositioning, positioning, and changing diapers as needed.

    • Support with mobility, including walking, transferring, and using mobility aids.

  • Medication Management:

    • Administer prescribed medications as per the schedule.

    • Remind family members in advance when medicines are running low to ensure timely refills and continuity of care.

    • Monitor for and report any side effects or medication-related issues to health manager and family

  • Vital Signs Monitoring:

    • Measure and record vital signs such as blood pressure, heart rate, temperature, and oxygen saturation.

    • Monitor and document changes in health indicators.

  • Nutritional Support:

    • Prepare and serve meals and snacks based on dietary restrictions and preferences.

    • Ensure proper hydration and nutrition throughout the day.

  • Emotional and Social Support:

    • Provide companionship and engage in activities that promote mental and emotional well-being.

    • Address any fears, concerns, or emotional needs of the patient.

  • Document Care Activities:

    • Maintain detailed and accurate records of care provided, including vital signs, medication administration, meals served, and patient observations.

    • Track and document symptoms and changes in the patient’s condition as outlined in the care plan

  • Safety and Emergency Preparedness:

    • Maintain a clean and sanitary living environment.

    • Schedule reminders for the regular changing of catheters and RT tubes based on care plan.

    • Check the RT tube regularly for any signs of blockage or leakage and ensure that it is functioning properly.

    • Immediately report any displacement of the Rt tube to the health manager and family members.

    • Document any changes in wound condition and report to the healthcare team.

    • In case of an emergency, promptly notify the patient's family and assist them in seeking emergency medical support.

  • Communication and Reporting:

    • Communicate regularly with the patient’s family and healthcare team.

    • Provide updates on the patient’s condition and any notable changes or concerns.

  • Support with Rehabilitation:

    • Assist with prescribed therapeutic exercises and rehabilitation activities as directed by healthcare professionals.

    • Facilitate participation in activities that improve physical and cognitive health.

  • Assist with Appointments:

    • Accompany patients to medical appointments and ensure they receive necessary follow-up care.

  • Overnight Care (if required):

    • Provide support during nighttime, including turning, repositioning, and managing incontinence if needed.
